When a pet is referred to our hospital or is our patient for any reason, we prefer to have that patient’s records so we can better understand the patient’s current condition and create an appropriate diagnostic or treatment plan. This may include records, labs results, radiographs, ultrasounds, MRIs, or CT scans, but our team will let you know if we need any specific test results or records for your patient or pet. If you are referring your patient to us, or if you need to submit your own pet’s records, please do so by uploading the documents below and completing the form with your contact information.
